The first half of the week we’re hosting Da Vinci Project demos with MultiCare Connected Care. We’ll be showing real-world use cases of Fast Healthcare Interoperability Resources (FHIR) supporting and integrating value-based care (VBC) data exchange across communities.
Data exchange can also streamline 30-day medication reconciliations, increasingly a requirement for VBC incentives. Siloed care settings present a unique problem for providers tasked with completing comprehensive medication records. Through our work with Washington State’s MultiCare Connected Care, we’ll share how FHIR enables auto-reconciliations directly integrated into the electronic medical record.
